Dont know whether i have a blighted ovum or fetal demise?

i went to the er and the dr did a ultrasound and said that i had a sac but it hadnt developed as it should he wrote on the form that it could be either fetal demise or blighted ovum, when can i really tell what is going on? could it be that i am having a miscarriage?
